Using the NEWS

When to use NEWS

 

All in-patients who have documented clinical observations should have a National Early Warning score recorded and logged on RIO. In addition a patient who is of concern to a health care professional should have a set of observations recorded and the score calculated and recorded on the NEWS observation chart and Rio Progress notes. Below are examples of situations when NEWS may be used (This is not an exhaustive list).

  • Patients whose condition is causing concern to be assessed
  •  
  • Patient with a chronic health problem with background information sought from the GP
  •  
  • Patient starting on Clozaril for pre and post administration monitoring
  •  
  • Patient recovering from ECT
  •  
  • Patient transferred from other hospitals/nursing home who require further monitoring
  •  
  • Patient who have received rapid tranquilisation via intramuscular route
  •  
  • Patient on end of life care pathway
  •  
  • New admissions

Score 1-2. Increase frequency of observations, inform nurse in charge (minimum 4 hourly Obs repeat)

 

Score 3 in one category Contact Senior Nurse, Duty Doctor Increase frequency of observations (minimum hourly observation and intermittent or continuous observation if required). Consider ABCDE

 

Score 4 and above Or increasing by 2, or AVPU score falls by 2 or Anyone whose condition is causing concern

Contact senior nurse/Assessor/duty doctor and summon emergency support.

Engage in resuscitation in accordance with BLS/AEB training and policy if required

Increase frequency of observation
